The Yellow Dot Program alerts first responders at crash scenes of critical medical information that could save a life. Participants help emergency responders make split-second decisions by placing a completed Yellow Dot personal information form into the glove compartment of the vehicle and the Yellow Dot decal in the lower-left corner of the driver-side rear window.
Materials are FREE for Kentucky residents. Each set costs nearly $2.00 to print and mail. By ordering only what you need, you’re helping us serve more people across the state.

Yes, you can get Yellow Dot materials for every vehicle you drive. If you need more than three, please email the service account
Yes, you should complete the personal information form for any other driver(s) or passenger of your vehicle, especially if they have a specific personal need (i.e. autistic, difficulty communicating in stressful situations, medical allergy, diabetes, heart problems).
How to participate: Place the Yellow Dot sticker on the lower-left corner of your vehicle’s rear window (driver’s side). Complete the personal information form and keep it in your vehicle's glove box. Each driver and passenger should have their own form stored in the glove box.
For motorcycles - Place the decal on the "triple tree" of the motorcycle and put the form in the saddle bag/compartment.
The photo is crucial to the success of the program. Yellow Dot is about saving those precious seconds immediately following a serious incident. Having a photo identifying to whom the personal information form belongs will save time and possibly your life. Please make sure a photo(s) is included with each personal information form.
Because it is kept in your glove compartment, only you have access to the information. The information is not kept by any agency or another person.
No, do not include social security number, credit card information or any other sensitive information.

TIP: We recommend having an event or segment of an event where participants actually fill out the brochure/personal information form and then everyone goes to their vehicles and adheres the decal on the rear windshield.
